On Friday, July 2, 2021, I was asked to do a direct pump off from the truck to the tank. I proceeded by using the two holes that are located on the side of the truck. I also useused the adapter that is located in the storage. I connected the holes to the back of the truck, where it says direct pump off. Toto the tank. When I turnturned on the pump, it started to leak where I clampclamped down the bunny ears. I shut off the pump Immediately Andimmediately and began to clean up the spill. I was told by the manager to stop cleaning up and leave. I continuecontinued to clean and leaveleft when the leak iswas resolved.
